Once the appointment has been obtained, a payment of 50 US dollars or its equivalent in Gourdes at the daily rate must be made to the Capital Bank.

The "Covid-19 Traveler Exam Request" form, available at https://gheskio.org/travel-form, must be duly completed and brought to the appointment.

The traveler will present himself at the collection center with the receipt from the Capital Bank and the completed form.

The sample must be taken within 72 hours before boarding the traveler.
